About The Results Package

The results package for each location screened contains a variety of reports. Most importantly, the package includes individual results sheets for each child screened with their screening images attached. Doctor response forms are also provided for children recommended for further evaluation. Summary booklets are included for use by school personnel.

Individual Results Sheets
Regardless of whether a problem is noted or not, each child screened has an individual results sheet returned with their screening picture attached to it. For results indicating possible problems, further explanation is provided on each sheet along with any follow-up recommendations.

Doctor Response Forms
Included with results sheets that recommend examination by an eye doctor is a doctor response form. The parent is asked to have their eye doctor fill out and return the form to Vision Research.

Summary Booklets
A summary booklet is returned for every screening location. These booklets contain an overall statistical breakdown of the screening which includes various totals helpful for school health reports, a total listing of all children screened at that location, and a summary of children noted with possible problems.
